Internal Medicine
Just adults — chronic care, done fast. Stack a patient’s conditions and chart real multimorbidity in one note.
What’s inside
Chronic-condition stacking
Freely stack a patient’s conditions — they merge into one coherent note, not three separate ones.
Acute complaints
Templates for the common adult presentations, everything defaulting to normal and “denies.”
Auto BMI & vitals
Enter vitals once; BMI computes itself, with unit conversion.
Allergy tracking
A structured allergy module carried into the note.
Patient summaries
Plain-language after-visit summaries — the concierge touch DPC members appreciate.
Custom builder
Add your own options or build whole templates for the visits you do — saved on your device.
Every visit, from the same clicks
Tap through the findings — SwiftChart writes the rest
Why not an AI scribe?
The fair question: why not just use one of the ambient AI scribes? Because SwiftChart works in a fundamentally different way — and that difference is the whole point.
Before you sign
SwiftChart doesn't just write your note — it checks that it's ready before you sign.
A second set of eyes, every note.
It never writes the note or makes the clinical call — it surfaces what a busy day can miss, so you review, correct, and sign with confidence.
Questions
I also see some kids / do wellness visits — should I get this?
If you see across the lifespan, Family Medicine is the superset — it includes everything here plus pediatrics, women’s health, and geriatric care. Internal Medicine is for adult-only practices.
Can I use it with real patient data yet?
You start on test data. The moment your Business Associate Agreement is executed, you’re cleared for real protected health information. It’s a deliberate, honest boundary — not a limitation you’ll be stuck behind.
Are the templates locked to how you built them?
No — a custom template builder lets you add your own click-options to any section, or build entire templates from scratch, for exactly the visits you do. They save on your device.
Am I locked into a contract?
No. It’s a short 1-month agreement, billed monthly via Stripe, and you can cancel anytime — effective at the end of the billing month, with no termination fee.
Adult chronic care, in clicks.
Introductory pricing covers your first month; the regular rate applies from month two onward.
