Get paid every month for the care you already coordinate.

Since 2025, Medicare pays primary care practices a monthly fee for Advanced Primary Care Management (G0556–G0558), up to about $117 per patient per month. No minutes to count. Most small practices haven’t started because nobody hands them the paperwork. This kit does.

The problem

APCM bundles care management into one monthly code, but CMS lists about ten required elements: consent with specific disclosures, an initiating visit, a shared care plan, 24/7 access, care transitions and more. CMS publishes the rules but no templates, and the paid help out there is mostly one-hour webinars.

This kit gives your team the forms, scripts and note templates to start enrolling patients this month, and a tracker that tells you who is ready to bill.

Questions

Is APCM worth it for a small practice?

Use the tracker’s revenue estimator with your own patient counts and local rates. As an example at 2026 national rates, 60 patients at Level 2 is roughly $3,200 a month before cost-sharing.

Do the rules change?

Medicare updates its fee schedule every year. The kit reflects rules checked in October 2026; before billing in a new year, check the final rule and the CMS APCM page.

Are the files editable?

Yes. Add your practice name, adjust the templates, and paste the smart phrases into any EHR.

Templates & education only. Not legal, billing, or compliance advice. Confirm current requirements with CMS and your Medicare Administrative Contractor before billing. Every note must reflect services actually provided.
